BARS — Behavioral Activity Rating Scale (Agitation)

Behavioral Activity Rating Scale Level

1 — Unarousable. Minimal or no response to noxious stimuli; does not communicate or follow instructions+12 — Very sedated. Arouses to physical stimuli but does not communicate or follow instructions; may move spontaneously+23 — Sedated. Difficult to arouse; communicates or follows instructions minimally; responds to stimulation+34 — Calm and cooperative. Awake; easily arouses; communicates; follows instructions+45 — Mildly agitated. Anxious or mildly agitated; attempting to sit up; calms down with verbal instructions+56 — Moderately agitated. Moderate agitation; trying to stand; requires gentle persuasion to calm; frequently distressed; not aggressive+67 — Dangerously agitated. Pulling at restraints, thrashing side to side, striking out; does not calm down despite restraints or verbal redirection+7
