The Hypoglycemia Risk Score is a clinical assessment tool designed to quantify the risk of severe and recurrent iatrogenic hypoglycemia in patients with diabetes mellitus, particularly those on insulin or secretagogue therapy. Severe hypoglycemia (blood glucose < 54 mg/dL or requiring third-party assistance) is the most dangerous acute complication of diabetes treatment, associated with significant morbidity including cardiac arrhythmias, falls, seizures, and excess mortality (ACCORD trial).
