The Modified Rankin Scale 9-Question (mRS-9Q) is a validated patient-reported outcome measure that assesses functional disability in patients with neurological conditions (particularly stroke, TBI, subarachnoid hemorrhage). The mRS-9Q consists of 9 yes/no questions about specific functional abilities that algorithmically map to an mRS score (0–6), providing a reliable surrogate for in-person structured interview administration. It allows remote or self-administered mRS assessment — important for clinical trials, telehealth follow-up, and mass casualty scenarios.
Q1: Do you have any symptoms or problems that are related to your neurological condition? (Stroke/brain injury symptoms)
Q2: Do you have any difficulty with your usual activities (work, hobbies, social activities)?
Q3: Are you able to look after your own affairs (finances, decision-making) without assistance?
Q4: Can you walk without assistance from another person? (Cane or walking frame/walker is okay — just no person-hold)
Q5: Can you go to the toilet and attend to your bodily needs without assistance from another person?
Q6: In the past week, have you spent the majority of the day in bed or in a chair (not walking around)?
Q7: Do you require constant nursing care and attention?
Q8: Overall, are there activities that you are no longer able to do that you previously could do?
Q9: Before this neurological illness, were you fully independent (mRS 0–1)?
