Ottawa Subarachnoid Hemorrhage (SAH) Rule for Headache Evaluation

Rules out subarachnoid hemorrhage in neurologically intact adult patients presenting to the ED with a new severe headache reaching maximum intensity within one hour of onset.

Parameters

Ottawa SAH Criteria (ANY present = SAH cannot be ruled out — further workup required)any

Age ≥ 40 yearsNeck stiffness or neck pain at presentationWitnessed loss of consciousnessOnset with exertion (straining, coughing, sexual activity)Thunderclap onset: pain reached maximum intensity instantaneously (within 1 second)Limited neck flexion on physical examination (pain or stiffness on passive flexion)
