RADS — Inhalation Injury Severity Score (Bronchoscopic Grading)

Classifies the severity of inhalation injury in burn and smoke inhalation patients based on direct bronchoscopic visualization of airway findings. Provides an objective grade (0–4) that correlates with ventilator days, ICU length of stay, and mortality. Guides airway management and intensive care decisions.

Parameters

Bronchoscopic Inhalation Injury Grade

Grade 0 — No Injury: Normal airway mucosa; no carbonaceous deposits, erythema, edema, bronchorrhea, or obstruction+0Grade 1 — Mild: Minor or patchy areas of erythema; carbonaceous deposits in proximal airways (above carina and in main bronchi)+1Grade 2 — Moderate: Moderate erythema, carbonaceous deposits, bronchorrhea; injury extends to the subsegmental level in larger bronchi below the carina+2Grade 3 — Severe: Severe inflammation with mucosal fragility; copious bronchorrhea and secretion burden; injury present in smaller, subsegmental airways+3Grade 4 — Massive: Evidence of mucosal sloughing, necrosis, and endoluminal obstruction; carbonaceous casts in smaller and subsegmental airways; near-complete airway involvement+4
