ROSE Rule for Syncope

The ROSE (Risk stratification Of Syncope in the Emergency department) rule identifies ED syncope patients at 1-month risk of serious outcomes (death, return ED visit for serious condition, or need for serious intervention) using a rapid assessment of 6 variables.

Parameters

ROSE Criteria (any single criterion = high risk)any

BNP ≥ 300 pg/mLB-type natriuretic peptide at ED presentation.Bradycardia on BBB on ECG (new or not previously known)Bradycardia ≤50 bpm or new bundle branch block on ED ECG.Fecal Occult Blood PositivePositive stool occult blood test at ED presentation.Hemoglobin ≤ 90 g/L (9 g/dL)Anemia on admission CBC.SpO₂ < 94% on Room AirOxygen saturation less than 94% on room air at rest.Q Waves on ECG (new or not previously known)Pathological Q-waves on ECG not attributed to known prior MI.
