ATLS Hemorrhagic Shock Classification

The ATLS (Advanced Trauma Life Support) Hemorrhagic Shock Classification stratifies hemorrhagic shock into four classes based on estimated blood loss, heart rate, blood pressure, respiratory rate, and mental status. Developed by the American College of Surgeons and integral to ATLS training, the classification guides initial fluid resuscitation, blood product transfusion, and surgical hemorrhage control decisions in trauma patients.

Parameters

ATLS Hemorrhagic Shock Class (Select the Best-Fitting Class Based on All Parameters)

Class I — Blood loss <750 mL (<15%). HR <100 bpm. SBP normal. Pulse pressure normal. RR 14–20/min. Mental status: alert, normal. Urine output >30 mL/h.+1Class II — Blood loss 750–1500 mL (15–30%). HR 100–120 bpm. SBP normal. Pulse pressure decreased. RR 20–30/min. Mental status: anxious. Urine output 20–30 mL/h.+2Class III — Blood loss 1500–2000 mL (30–40%). HR 120–140 bpm. SBP decreased. Pulse pressure decreased. RR 30–40/min. Mental status: confused. Urine output 5–15 mL/h.+3Class IV — Blood loss >2000 mL (>40%). HR >140 bpm. SBP severely decreased. Pulse pressure very decreased. RR >40/min. Mental status: lethargic/unconscious. Urine output negligible (<5 mL/h).+4
