SLICC 2012 Classification Criteria for Systemic Lupus Erythematosus

The Systemic Lupus International Collaborating Clinics (SLICC) 2012 classification criteria for SLE. An update to the ACR 1997 revised criteria, with improved sensitivity (97% vs. 83%) at the cost of slightly lower specificity (84% vs. 96%). Classification requires ≥ 4 criteria (≥ 1 clinical and ≥ 1 immunological) OR biopsy-confirmed lupus nephritis with ANA or anti-dsDNA positivity.

Parameters

Clinical Criteria (≥ 1 required for classification)

1. Acute Cutaneous LupusButterfly (malar) rash; lupus erythematosus profundus; mucosal lupus erythematosus; toxic epidermal necrolysis-like SLE; UV light-sensitive rash; maculopapular lupus rash.2. Chronic Cutaneous LupusClassic discoid rash; hypertrophic lupus; lupus panniculitis; mucosal lupus; lupus erythematosus tumidus; chilblains lupus; discoid lupus/lichen planus overlap.3. Oral or Nasal Ulcers (Painless — unless Painful = Aphthous)Painless oral or nasal ulcers in the absence of other causes. Aphthous ulcers (painful) count if no other explanation.4. Non-Scarring Alopecia (Diffuse Thinning)Diffuse hair thinning without scarring (not attributable to other causes).5. Synovitis (≥ 2 Joints, Swelling or Effusion)Synovitis in ≥ 2 peripheral joints characterized by swelling or joint effusion OR tenderness in ≥ 2 peripheral joints and ≥ 30 minutes of morning stiffness.6. Serositis (Pleuritis or Pericarditis)Typical pleurisy for > 1 day, OR pleural rub, OR pleural effusion on imaging. OR Typical pericardial pain (worse lying flat), OR pericardial rub, OR echocardiographic effusion.7. Renal InvolvementUrine protein-to-creatinine ratio (or 24h urine protein) representing ≥ 500mg/24h, OR RBC casts.8. Neurological CriterionSeizures; psychosis; mononeuritis multiplex (NOT due to other causes); myelitis; peripheral or cranial neuropathy (NOT due to other causes); acute confusional state (NOT due to other causes).9. Hemolytic AnemiaPositive Coombs' test (direct antiglobulin test) with hemolytic anemia.10. Leukopenia or LymphopeniaLeukopenia < 4.0 × 10⁹/L at least once (NOT due to other causes) OR Lymphopenia < 1.0 × 10⁹/L at least once (NOT due to other causes).11. Thrombocytopenia (< 100 × 10⁹/L)Platelet count < 100,000/µL at least once (NOT due to other causes — exclude drugs, ITP, TTP).

Immunological Criteria (≥ 1 required for classification)

12. ANA (Antinuclear Antibody) PositiveAbove laboratory reference range.13. Anti-dsDNA PositiveAbove laboratory reference range (except ELISA: 2× above normal).14. Anti-Sm (Anti-Smith) Antibody PositivePresence of anti-Sm antibody — highly specific for SLE.15. Antiphospholipid Antibody PositivityPositive lupus anticoagulant, false-positive RPR, medium-/high-titer anticardiolipin (IgA/IgG/IgM), or anti-β2-glycoprotein I (IgA/IgG/IgM).16. Low ComplementLow C3, Low C4, or Low CH50 (total hemolytic complement).17. Positive Direct Coombs Test (Without Hemolytic Anemia)Positive direct Coombs test in the absence of hemolytic anemia (hemolytic anemia counted under criterion #9 instead).
