Vancouver Chest Pain Rule

Identifies chest pain patients who are low risk and safe for early discharge without further evaluation for ACS.

Parameters

High-Risk Clinical Features (ANY present = NOT low risk)any

Known coronary artery disease (prior MI, PCI, CABG, or angiographically confirmed CAD)Diaphoresis present at evaluationPain radiates to the arm or shoulderPain reproduced or worsened by exertion

ECG Findings (ANY present = NOT low risk)any

Ischemic ECG changes (new ST depression, ST elevation, T-wave inversion, or new LBBB)

Troponin Results (ANY present = NOT low risk)any

Initial troponin (0h) elevated above institutional normal threshold2-hour troponin elevated or significant delta rise (≥3 ng/L absolute rise for hs-TnT)
