Diagnosis explanation
Second-degree AV block (Wenckebach / Mobitz I) means the electrical signal from the upper to lower heart chambers is periodically delayed, causing occasional "skipped" or "dropped" heartbeats
This is usually a benign condition, especially when due to medications, sleep, or athletic training
In most cases, this does not require a pacemaker
Treatment depends on whether it is causing symptoms and what is causing it
Medication instructions
If a medication was adjusted or stopped, do not restart without physician advice
Do not abruptly stop beta-blockers — risk of rebound cardiac events
Continue all other medications as prescribed
Bring a complete medication list to all follow-up appointments
Activity guidance
Athletes with asymptomatic Mobitz I and normal heart structure may continue all competitive sports
Per AHA/ACC 2025 scientific statement — no restriction if no haemodynamic compromise
Avoid driving or operating heavy machinery until follow-up if syncope or near-syncope has occurred
Report symptoms to physician before resuming high-risk activities
Return to emergency department instructions
Return immediately for any of the following
Fainting, loss of consciousness, or near-fainting episode
Severe dizziness, lightheadedness, or feeling that heart has stopped
Chest pain, pressure, or tightness
Shortness of breath at rest or worsening dyspnoea
New or worsening palpitations or irregular heartbeat
Prolonged pauses felt in the heartbeat
Call emergency services (911) for loss of consciousness or chest pain
Follow-up plan
Cardiology outpatient appointment scheduled within 1-4 weeks
Bring all ECGs and hospital discharge paperwork
A heart monitor (Holter or event recorder) may be arranged to capture rhythm
Wear monitor as instructed and keep symptom diary
